Hey there!
I played the Paramedics missions in Vice City to replicate your bug. Unfortunately I wasn't able to.
I am on 4.1.7 as well, have a original 1.0 non steam version and played in Vice City.
Perhaps you are on a steam version and this is why it crashed? Otherwise I wouldn't see a reason since it worked fine until level 12 for me.
